For more information and guidance on using the online services, please contact Grasmere Surgery and we will help you set yourself up to use it, and how to obtain your username and password.

Grasmere currently offers the facility to patients: –

  • to book, view, amend, and cancel appointments online.
  • to order online, view and print a list of their repeat prescriptions for drugs, medicines or appliances.
  • to view online, export or print summary information from their record, relating to medications, allergies, adverse reactions and any other items agreed between the practice and individual patient.
  • to view detailed coded records.
  • Available 24/7.

For access to detailed coded records, if you are registered for Online Access. you will still need to read the access information leaflet and complete an application form. Once this has been approved you will be able to access your records within 14 days

Grasmere Surgery now uses an online form called SystmConnect available through SystmOnline (at the bottom of SystemOnline webpage), as the main way to access our services. SystmOnline – how to use

Patients who don’t have internet access, or someone who can help them online, can still phone the surgery or visit.

Why We’re Using the Online Form

Using the online form helps us:

  • Provide quicker access to the help you need.
  • Reduce the number of phone calls you need to make.
  • Direct your request to the most appropriate clinician (GP, Advanced Clinical Practitioner, or Pharmacist).
  • Use our limited NHS resources more effectively.

Patients also have the option to book routine appointments in advance.

If you are being treated for an ongoing issue, please tell us who has been dealing with your care so we can ensure continuity.

The form is quick and easy to complete — but please include as much detail as possible.

Forms with limited information may need to be returned for clarification.

Prescriptions and Results

You can continue to order repeat prescriptions via:

  • The NHS App
  • SystmOnline
  • We do not take repeat prescriptions over the telephone.

You can also view your test results through the NHS App without needing to contact the surgery.

We strongly encourage all patients to sign up for the NHS App.

If you haven’t already, please register to receive SMS messages — this allows us to contact you quickly about appointments, results, and other updates.

Appointment System FAQs

1. What online forms are available?

You can choose between:

  • Medical Request.
  • Admin Request.

Admin Requests include:

  • Fit (sick) note requests.
  • Non-medical queries (e.g. referrals or information).

Medical Requests include:

  • New conditions – routine not emergencies.
  • Existing conditions – routine not emergencies.
  • Medication queries – routine not emergencies.

You will also be asked for additional details such as times when you are not available during surgery hours.

Please note the medical guidance given by the System before submitting a form e.g. ring 111.

2. Primary Care – Get to know where to go

Before submitting your request, please look at the below information i.e. do you know that many local pharmacies can now treat some common health problems and provide prescription medicine if needed – with no GP appointment needed.

  • Earache (aged 1 to 17).
  • Impetigo (aged 1 year and over).
  • Infected insect bites (aged 1 year and over).
  • Shingles (aged 18 years and over).
  • Sinusitis (aged 12 and over).
  • Sore Throat (aged 5 and over).
  • Urinary Tract Infection (UTI) women only (aged 16-64).

Did you know that many minor ailments can be seen by your local Pharmacy:

  • Coughs, colds and sore throats. tummy upsets.
  • Aches and pains.
  • Skin rashes.
  • Baby teething.
  • Nappy Rash.
  • Emergency contraception.
  • Free blood pressure checks.

3. When can I submit a form?

Forms for routine medical or general enquiries can be submitted:

  • Monday to Friday, 8:00 am – 6:30 pm

Outside these hours:

  • Contact NHS 111 if your problem cannot wait.
  • Call 999 if it’s a medical emergency.

You can still:

  • Book online appointments.
  • Order repeat prescriptions.

These services are available 24 hours a day, 7 days a week.

4. What if I consider my problem as a Medical Emergency?

Grasmere Surgery is not an emergency service.

If you think you may be experiencing a stroke, heart attack, seizure, or another serious condition, please call 999 immediately.

5. How do I request a repeat prescription?

You can:

  • Submit a paper request.
  • Use the NHS App or SystmOnline.

Note: we do not accept prescription requests by phone.

6. Can I still see my usual clinician?

Yes. You can request to be dealt with by your usual clinician, and we’ll do our best to accommodate your request depending on the Doctors leave etc.

7. What if I don’t have access to a smartphone or computer?

Most patients have internet access or can ask a relative or carer to assist them.

If that’s not possible, one of our receptionists can take your details over the phone and complete the form for you, or a family member, carer or trusted friend can have proxy access – please speak to our reception team for further information on proxy access.

FYI – Proxy Access allows someone on your behalf to book appointments, order your medication, and send online requests, once the patient gives consent.

8. How long will it take to get a response?

Our reception team reviews online consultations throughout the day. We aim to respond within one working day, However, this may change during a Pandemic.

9. What if I don’t want to give details of my problem?

We ask that you provide as much detail as possible so we can triage your request safely and effectively.

Forms marked “personal” or lacking information may be returned for clarification, which could delay your care.

10. What if I have more than one issue?

Please submit as much information about each problem. This ensures each problem receives the appropriate attention.

11. What information is need to complete the form

The forms will self populate with the patients details, and these forms as for the patient only, not another person. you will then be prompted to fill in the rest of the questions e.g. how would you like a reply?

12. How do I request a home visit?

Please call the surgery before 12:00 pm if you need to request a home visit.

13. What if I don’t like the new system?

We understand that changes can take time to get used to. However, we’re confident this new approach will be more efficient and convenient for most patients.

With increasing demand and limited NHS resources, online triage systems are becoming standard practice across GP surgeries nationwide.

14. Is this system NHS-approved?

Yes.
Patient Triage has been fully assessed by NHS Digital and meets all requirements under the GP IT Futures framework, including full GDPR compliance.

16. How do I sign up to use the Online services

If you are not registered for online services, please visit our reception and bring in one of the following: Photographic ID: Passport or Photo Driving Licence. If you telephone for access, you may be asked security questions before online access is approved.
